Ipamorelin vs Sermorelin: What's the Difference?

Sermorelin mimics GHRH and Ipamorelin mimics ghrelin. Both prompt the pituitary to release growth hormone, but Sermorelin has more human data and a clearer prescription path.

Key Takeaways

  • Sermorelin is a 29-amino-acid copy of the active portion of growth-hormone-releasing hormone (GHRH). Ipamorelin is a five-amino-acid peptide that acts on the ghrelin receptor.
  • Both signal the pituitary gland to release your own growth hormone (GH).
  • Sermorelin was once FDA-approved as Geref and was later discontinued for reasons the FDA determined were unrelated to safety or effectiveness. Ipamorelin has never been FDA-approved.
  • FDA lists Ipamorelin acetate among compounding substances that may present significant safety risks. Sermorelin is compounded by licensed pharmacies with a prescription.
  • The strongest human data for Sermorelin-class peptides come from an injection trial in adults aged 55 to 71. Ipamorelin's human data come mainly from intravenous (IV) studies.

Sermorelin mimics growth-hormone-releasing hormone (GHRH), and Ipamorelin mimics ghrelin. Both prompt the pituitary to release growth hormone, but Sermorelin has more human data and a clearer prescription path.

Growth hormone is released in pulses, and the largest pulses come during sleep. Clinicians track it through insulin-like growth factor 1 (IGF-1), measured with bloodwork.

Rebody clinicians prescribe Sermorelin as an injection, nasal spray, or sublingual troche.

Sermorelin pros: FDA approval history, more human data, and a legal method through licensed compounding pharmacies and needle-free formats.

Sermorelin cons: Little head-to-head absorption data for the troche or nasal spray. In the main older-adult trial, sleep quality did not change, and lean mass increased in men only.

Ipamorelin pros: In animal studies, it released GH without raising the stress hormones ACTH or cortisol, unlike older peptides in its class.

Ipamorelin cons: Never FDA-approved, limited human efficacy data, and FDA safety concerns for compounded versions.

How Ipamorelin and Sermorelin Work

Both peptides signal the pituitary to release GH through different receptors.

How Sermorelin works

Sermorelin is a 29-amino-acid analog of endogenous growth-hormone-releasing hormone. It binds to the GHRH receptor on pituitary somatotroph cells and amplifies the GH pulse your brain is already sending. Your body's feedback loop still regulates how much GH is released.

Sermorelin isn't the only GHRH analog. Tesamorelin is FDA-approved as Egrifta for a specific condition in adults with HIV, and CJC-1295 is a longer-acting analog that has never been approved.

How Ipamorelin works

Ipamorelin is a pentapeptide that binds selectively to the ghrelin receptor (also called the GH secretagogue receptor, or GHS-R), which triggers a GH pulse from the pituitary.

In a 1998 study in pigs, Ipamorelin released GH without raising ACTH or cortisol, even at very high doses, while the older peptides GHRP-2 and GHRP-6 raised both. That selectivity was shown in animals; the main human study measured GH, not cortisol.

What the clinical evidence shows

Human data come from small studies: one Sermorelin-class injection trial and two IV Ipamorelin studies.

Growth hormone and IGF-1 response

In a 16-week trial, 19 adults aged 55 to 71 took a close Sermorelin analog by injection. GH rose within minutes of each injection and stayed elevated for about two hours, and IGF-1 increased within two weeks, then drifted back toward baseline by week 16.

For Ipamorelin, a 1999 pharmacology study gave healthy men Ipamorelin as a short IV infusion. GH peaked at about 40 minutes and then fell to negligible levels, and Ipamorelin's half-life was about two hours.

Neither study tested a troche or nasal spray.

Body composition and sleep

In the same older-adult trial, lean body mass rose in men but not in women, and self-reported sleep quality did not change in either group. No published human trials have tested Ipamorelin for body composition or sleep in healthy adults.

Side effects and safety

Sermorelin can cause headache, flushing, dizziness, nausea, or sleepiness in any format. The injection can also cause pain, redness, or irritation where it's given; the nasal spray, nasal irritation, a runny nose, or an unusual taste; and the troche, mouth or tongue irritation or an unusual taste. In the older-adult trial, the only adverse effect was a temporary rise in blood lipids that resolved by the end of the study.

For Ipamorelin, the largest human trial gave IV Ipamorelin to 114 adults for up to seven days after bowel surgery. Because these were patients recovering from major surgery, most people in both groups had at least one adverse event (87.5% with Ipamorelin vs. 94.8% with placebo). The drug also did not significantly shorten the time to a first tolerated meal. Low potassium, high blood sugar, and trouble sleeping were more common with Ipamorelin than with placebo. For compounded versions, the FDA cites possible immune reactions and peptide impurities.

Growth hormone peptides aren't prescribed during pregnancy or breastfeeding, for anyone under 18, or for anyone with active cancer or a history of cancer. Both peptides are banned for tested athletes under section S2.2.4 of WADA's 2026 Prohibited List.

Sermorelin formats: injection, nasal spray, or sublingual troche

Rebody clinicians prescribe Sermorelin, and it comes in three formats. The molecule is the same; the package and the trade-offs differ.

Format What's in the package Trade-off
Injection 5 mL vial, 3 mg/mL (15 mg total) The most-studied format; uses a needle
Nasal spray 6 mL bottle, 1,000 mcg/mL (6 mg total) Needle-free; can irritate the nose
Sublingual troche 1 mg per troche Needle-free; little head-to-head absorption data compared with injection

Your clinician sets the dose and schedule, and your prescription label carries them.

Sermorelin itself is well studied, mostly as an injection. The troche and nasal spray are newer, needle-free ways to take it, and there isn't much head-to-head data on how well they absorb compared with injection. Work with your provider to track how you respond over time, including changes in your bloodwork.

Compounded medications are not FDA-approved.

Who is a good candidate for Sermorelin or Ipamorelin?

A clinician decides based on your symptoms, history, medications, and labs.

Adults noticing changes in sleep and recovery

GH secretion falls from early adulthood to midlife, alongside a drop in deep sleep. If your recovery is slower or your sleep is lighter, a clinician can check IGF-1 and your history before deciding whether Sermorelin fits.

People who would rather skip needles

The nasal spray and troche remove the syringe. A clinician can match the format to your schedule and travel.

Men already on testosterone therapy

If you take testosterone, tell your clinician before starting either peptide. Neither Sermorelin nor Ipamorelin has been studied in controlled trials as an add-on to testosterone therapy, so the decision rests on your labs and history.

For men who want to raise their own testosterone, Rebody prescribes Enclomiphene capsules, which work through the body's LH and FSH signal and start with bloodwork.

Regulatory status, cost, and safe access

Access is the biggest difference between the two, and it shapes what each costs.

Sermorelin cost compared to Ipamorelin

Only Sermorelin has a legal prescription path. Your clinician sets how long treatment lasts; the main older-adult Sermorelin-class trial ran 16 weeks.

Cost factor Sermorelin Ipamorelin
Prescription path Compounded by a licensed 503A pharmacy with a prescription Never FDA-approved; FDA lists it as a compounding safety risk
Price with Rebody Shown on each Sermorelin product page Not offered
What the price covers Provider review, medication, and free shipping, with no separate consult fee Not applicable

Ipamorelin vials labeled "laboratory-only" often cost less because they skip pharmacy testing, a prescription, and clinician review. They aren't made for human use. For Sermorelin, ask your clinician whether follow-up IGF-1 bloodwork adds to your cost.

Sermorelin's FDA history

Sermorelin acetate was FDA-approved as Geref, including a 1997 approval for children with growth failure caused by GH deficiency. The manufacturer discontinued it around 2008, and in 2013, the FDA determined Geref was not withdrawn for safety or effectiveness reasons. Licensed pharmacies now compound Sermorelin for individual patients with a prescription. Compounded Sermorelin is not FDA-approved.

Ipamorelin's compounding status

Ipamorelin has never received FDA approval. In September 2023, FDA placed Ipamorelin acetate in Category 2 for outsourcing facilities, and it appears on FDA's list of substances that may present significant safety risks in compounding, citing possible immune reactions and peptide impurities.

The 503A nomination for Ipamorelin was later withdrawn, and FDA's Pharmacy Compounding Advisory Committee voted against adding it to the 503A list in October 2024. Regulatory status can change, so a clinician should confirm the current status before any prescription.

How to compare peptide providers and products

Peptide search results mix telehealth clinics with vials labeled "laboratory-only." Check for:

  1. A licensed clinician who reviews your history and labs before prescribing.
  2. A licensed U.S. 503A compounding pharmacy that labels the medication with your name and strength.
  3. A legal compounding path for the peptide. laboratory-only vials aren't made for human use and skip pharmacy oversight.
  4. A price that covers provider review, medication, shipping, and follow-up.
  5. A care team you can reach about side effects, your prescription, and labs.

Unregulated peptides carry three specific risks: uncertain purity, inaccurate dosing, and contamination.

Choosing between Ipamorelin and Sermorelin with a clinician

Both peptides ask your pituitary for more growth hormone through different receptors. Ipamorelin's selectivity data come from animals. Sermorelin has an FDA approval history, injection data in older adults, and three prescription formats.

With Rebody, a licensed provider reviews your goals, history, and labs; a licensed U.S. compounding pharmacy prepares the format you choose; and follow-up tracks IGF-1, sleep, and recovery.

Schedule a consult with a Rebody clinician to see whether Sermorelin, as an injection, nasal spray, or troche, fits your routine.

Frequently Asked Questions (FAQs)

Does Ipamorelin work better than Sermorelin?

No study has compared them head-to-head. Sermorelin-class peptides have more human data, including a 16-week injection trial in older adults that raised IGF-1. Ipamorelin's human data come mainly from an IV study in healthy men and a trial after bowel surgery.

Does Sermorelin work the same as Ipamorelin?

No. Sermorelin mimics GHRH and acts on the GHRH receptor. Ipamorelin acts on the ghrelin receptor. Both lead the pituitary to release growth hormone.

What peptide is stronger than Sermorelin?

No study has tested which is stronger. Tesamorelin is a GHRH analog FDA-approved as Egrifta for a specific condition in adults with HIV. CJC-1295 is a longer-acting GHRH analog that is not FDA-approved. A larger GH signal doesn't mean a better outcome.

What are the downsides of Ipamorelin?

It has never been FDA-approved, and the FDA lists it as a compounding safety risk, citing possible immune reactions and peptide impurities. In an IV trial after surgery, low potassium, high blood sugar, and trouble sleeping were more common than with placebo. Human efficacy data are limited, and WADA bans it for tested athletes.

Can you take Sermorelin and Ipamorelin together?

No controlled trials have tested the combination for aging or recovery, and Ipamorelin's compounding status limits legal access. Any combination needs clinician review.

Is Sermorelin FDA-approved?

Sermorelin was FDA-approved as Geref for children with growth hormone deficiency and was later discontinued by its manufacturer. FDA determined the withdrawal was not for safety or effectiveness reasons. Compounded Sermorelin prescribed today is not FDA-approved.

Can you combine a GHRH analog with Ipamorelin?

Because they act on different receptors, some clinics pair a GHRH analog with Ipamorelin, most often CJC-1295. Neither that pair nor Sermorelin plus Ipamorelin has published controlled trials for aging, sleep, or recovery.
